Our Approach at Reform Physical Therapy

Prehabilitation begins 4 to 8 weeks before surgery. The goal is to maximize quadriceps and hip abductor strength, optimize pre-surgical range of motion, and educate patients on post-surgical precautions posterior hip precautions for posterior approach THR, and weight-bearing restrictions specific to your surgical protocol.

Phase 1 (weeks 0 to 6): Focuses on wound healing, edema management, early range of motion, and safe mobility. Target ROM benchmarks include 0 to 90 degrees TKR flexion by week 2. DVT prevention through early ankle pumps, compression, and monitored activity is integrated from day one.

Phase 2 (weeks 6 to 12): Progressive strength building, gait normalization, stair training, and return to community ambulation. Range of motion target for TKR is 0 to 120 degrees by week 6. Manual therapy addresses scar tissue and soft tissue restrictions limiting joint mobility.

Phase 3 (week 12 onward): Functional training, endurance development, and return to recreational activities. Dr. Watroba communicates written progress reports to your surgeon at 2 weeks, 6 weeks, and 12 weeks post-surgery.

Lauren Garcia, PTA contributes 10 years of hands-on manual therapy experience in outpatient orthopedics, supporting joint mobilization, scar management, and progressive exercise throughout all phases.

Clinical Tools & Assessment: Assessment tools include: goniometry (ROM benchmarks: 0-90 TKR week 2, 0-120 week 6), manual muscle testing, Timed Up and Go (TUG), KOOS (Knee Injury and Osteoarthritis Outcome Score), HOOS (Hip Disability and Osteoarthritis Outcome Score), and Western Ontario and McMaster Universities Arthritis Index (WOMAC).

Frequently Asked Questions

Prehabilitation is PT performed 4 to 8 weeks before surgery to strengthen surrounding muscles, optimize range of motion, and prepare you physically and educationally for surgery. Studies show prehab patients have shorter hospital stays and better functional outcomes at 3 and 6 months.

Most surgeons recommend starting PT within 24 to 72 hours of surgery, sometimes before hospital discharge. Early initiation produces significantly better range of motion and functional outcomes compared to delayed PT.

Outpatient PT provides specialized equipment, hands-on manual therapy, and direct clinical progression that home health cannot replicate. Patients who complete outpatient PT typically achieve better range of motion, strength, and return-to-activity outcomes.

Most patients resume light daily activities within 6 weeks. Driving typically returns at 4 to 6 weeks. Full recovery for demanding physical activities takes 3 to 6 months for daily function and up to 12 months for high-impact pursuits.

Yes. Medicare Part B covers outpatient PT following joint replacement when medically necessary. Our billing team verifies your specific coverage and any cost-sharing before your first visit at no cost to you.

Tell your therapist immediately. Plateaus are often caused by scar tissue, compensatory movement patterns, or insufficient progressive loading. We adjust the program, apply targeted manual therapy, and communicate directly with your surgeon if clinical concerns arise.
